Orders, Payments & Pricing
To make a purchase, simply add the product to your shopping cart and provide valid credit or debit card information on the Site's order page. Credit and debit card payments on the Site are processed by a third-party credit card processor, Authorize.net, which accepts the following credit or debit cards: [VISA, MASTERCARD and AMERICAN EXPRESS]. By submitting such credit or debit card information, you give Authorize.net permission to charge, on our behalf, your purchases to the card that you designate on the order page. We reserve the right to cancel any order at any time and for any reason.
If your credit or debit card has already been charged and your order is canceled, you shall receive a credit to your card account in the amount of the charge. Neither Covalent Medical, LLC nor Authorize.net will be responsible
(i) if your card issuer refuses or fails to authorize payment by you,
(ii) for any charges that your card issuer may apply to you as a result of the processing of your order, or
(iii) for any delays or non-delivery of products that may arise due to any validation checks that may be carried out regarding your payment or identification.
All orders are subject to availability. In addition, we reserve the right, at our sole discretion, to refuse or cancel any order for any reason. For your convenience, you will not be charged until your payment method is authorized, the order information is verified for accuracy and your order is shipped. We will contact you if all or any portion of your order is canceled or if additional information is required to accept your order. If your order is canceled after your credit or debit card has been charged, we will have Authorize.net issue a credit to your card in the amount of the charge.
The prices and availability of products displayed on the Site are subject to change without notice. In addition, pricing or typographical errors may occur and, in the event that a product is listed at an incorrect price or with incorrect information, we shall have the right, in our sole discretion, to refuse or cancel any orders placed for that item. If an item is incorrectly priced, we will either contact you for instructions or cancel your order and notify you of such cancellation.
Shipping & Auto-Ship Program
Products are shipped via UPS Ground, unless otherwise requested by you, and only to addresses within the United States. Shipping and delivery costs, which are not included in price, are set by UPS or other carriers and are your responsibility. Appropriate shipping charges will be added to the order total. No shipping is available to P.O. Boxes.
You may also join the auto-ship program, which is an optional feature we have added for the convenience of our customers. When you choose the auto-ship program, we will automatically ship you a three-month supply bottle every three months. Your credit card will be charged the cost of a three-month supply at the time you place the initial auto-ship order and every three months thereafter when we process your next auto-shipment, until you cancel your enrollment in the program.
You may cancel auto-ship at any time by calling (855) 659-9814 and requesting cancellation.. Your request will be processed immediately and you will not receive any automatic shipments of the product after the date of cancellation.
With respect to shipments of products to consumers, wherever they may reside, title to products will be shipped FOB the Covalent Medical, LLC plant and all risk of loss shall pass to you immediately upon our delivery of the products to the common carrier. You are solely responsible for providing us with accurate shipping information and a valid phone number or email address. In the event that any product is returned to us as a result of your failure to do so, you shall be responsible for any additional shipping fees or other charges that may be applied to the return of the product to us or in re-sending the product to you. Please note that refunds will not be issued for any order that is refused or abandoned by the intended recipient, orders that can't be delivered due to an address error, or orders seized by customs.
Cancellations, Returns, and Refunds
You may terminate an order in whole or in part upon notice to us within 24 hours after order is placed. You shall have the right to return defective goods within five (5) days for replacement or credit only. All claims of defective goods must be reported to Covalent Medical, LLC within five (5) days of receipt. Retail purchases may be returned for a full refund less shipping and handling within thirty (30) days of purchase.
Retail returns require an RMA number. The product ship date is the date of purchase by the consumer. You will receive a full refund less shipping and handling for unopened items in the original form of payment and merchandise credit less shipping and handling for opened items. Wholesale orders are not refundable except in the case of damaged goods.
Use of Products
All retail size products are for retail sale only at your place of business.
All professional size products are for use exclusively in your treatments and services, and cannot be sold.
You acknowledge that, as between the parties, Covalent Medical, LLC owns all rights, title and interest in the intellectual property within the products sold on the Site.
